It's kind of a cliché thing to say, but it's also true. Unless you've lived there, you don't understand it. I'm pretty familiar with college sports and have been several places. Rivalries like Auburn v Alabama and Ohio State v Michigan are probably more important because they oftentimes directly effect the national championship picture. Louisville vs Kentucky isn't even a conference game, so as far as what it means on paper, it's just a showcase game between two tournament caliber teams where the winner gets credit for a good win and the loser isn't hurt all that badly.
Having said that, as far as the off-paper portion of the rivalry, I've never been around two fanbases that hated each other more than these two do. It may not be the biggest rivalry and it may not have high stakes on paper, but I do think it is the most bitter rivalry. I grew up a Louisville fan. I don't really root for them anymore, but I still hate Kentucky. I'm still anxious to see this movie, though.
